Attention Seniors!!!! Diploma Name Verification: Seniors will need to login (link below) with their school credentials and complete the Diploma Name Verification Form. Parents/guardians cannot complete this form as they do not have login capability. The names are based on what has been submitted into Power Schools during enrollment. Diplomas are legal documents, so nicknames cannot be used. Any name change must be completed through the Registrar with appropriate legal documentation. Common errors found in the past are suffix and middle name omissions and special characters that do not transfer between programs (á, é). Any spelling errors should be addressed with your Counselor/Registrar as well as completing the form. Please complete by Wednesday, November 30. If form is not completed, the name as it is in Power Schools will be used. Seniors will be responsible for purchasing a corrected diploma if an error occurs due to failure to complete form or if the Senior submits a correction that is an error as well.
